Please go through the Notification published on university website before filling the form.
Pay the separate reappear fee for each semester and submit the reappear form for each semester.

Get ready with the Transaction ID/UPI, Transaction Document in pdf Format
Get ready with Passport Size Photo and Scanned Signature in JPEG/PNG format.
Do not repeat the Transanction ID/ Document If, any repeated information found the reappear form will be rejected.
Admit card will be generated only after verification of the fees submitted.
Keep the filled application form in your record after applying the reappear.
